We provide occupational therapy services for older adults at home, across Perth. We support clients with Home Care Packages and My Aged Care Funding, White and Gold Card Veterans and privately funded occupational therapy appointments. Services include:

• Equipment Prescription – Working with our equipment supplier network, we provide expert recommendation and prescriptions for equipment such as wheelchairs, walkers, transfer devices and kitchen equipment to enhance daily living activities.
• Home Modifications - Our occupational therapists complete home assessments and supporting recommendations for home modifications, including access changes, rails, ramps, doorways and bathroom modifications, to help you or your family members stay independent at home.
• Comprehensive Assessments - We conduct assessments, home environment assessments and assistive technology (AT) assessments, aligned with your Home Care Package to determine the best solutions for an individual’s needs.
• Collaboration with Home Care Package Managers - We work under your Home Care Package Manager to coordinate services. We ensure that supports provided align with your care plan and budget, ensuring effective service delivery.

Home Modifications Assessments

During a home visit, our Aged Care Occupational Therapists thoroughly assess your living environment to identify any areas that may benefit from modifications. These assessments aim to enhance safety and accessibility, enabling you to navigate your home safely and with ease. Based on the assessment, we may recommend various modifications such as:

• Installation of handrails and grab bars,
• Widening doorways for easier access,
• Adding ramps for wheelchair accessibility,
• Bathroom modifications to ensure safety during personal care activities.

Our Occupational Therapists provide clinical assessments, expert advice, and product recommendations tailored to your specific needs. This includes seating and positioning systems, mobility aids, and other assistive technologies. We consider various factors during the assessment, such as:
